There are two basic 'styles' of forms we use to in the Auxiliary document our activities. The most common form are the "fill-in the blanks" pdf forms.  These forms prompt you for the correct input, provide error checking, and allow you print out the form for mailing to the FSO-IS and are identified below with the blue 'pdf' link.  Just click on the link and the form will open on your screen.  You may also download the form using your right mouse button to save the blank form on your own computer.

Don't like dealing with paper?  Try the e-file style form.  These are also pdf  fill in the blank forms but with an added SUBMIT button and a place to enter a couple of email addresses.  Fill out the form like you normally would, add the FSO's email address in the "recipient" field, your email, and hit the SUBMIT button.  Make sure you are connected to the internet before hitting the SUBMIT button.  Give it a few seconds and your report will be forwarded directly to the FSO-IS.  This is confirmed with a printable confirmation page you can print a save if you wish.

The ever popular 7029 form is also available in MSWord format which allows saving incomplete forms to be added to as time goes along.  Suggestion: Put a fresh, blank 7029 form on you computer desktop.  There, it is available with a single click of the mouse, making it very convenient to keep up to date as you log hours.  When its time to file it with the FSO-IS you can print it out and drop in the mail or just attach the completed form to an email.

Of course, filling out paper version of the forms and mailing them in is also perfectly acceptable.
